Masks and Face Coverings
We require ALL patients to please wear a mask that covers their mouths and nose at all times while in our office. This is for the health and safety or our staff and the other patients. This is regardless of vaccination status.
Please keep your mask on while in our office, including the waiting area.

COVID-19 Questionnaire
Please answer these questions before coming to your scheduled appointment with us:
- Have you been exposed to anyone who is COVID-positive in the past 10 days?
- Are you experiencing elevated temperature or feeling feverish?
- Do you have a sore throat, sneezing, or cold/flu-link symptoms?
- Have you tested positive for COVID within the last 10 days?
If you have answered yes to some or all of these, then please reschedule your in-person appointment at a later date, or consider a virtual telehealth visit with one of our providers.
